Physicals

• All Freshman/Transfer student-athletes must complete a new physical. Physical MUST be completed using this form (we will not accept any other physical form). Must be completed AFTER May 1, 2025.

ADD/ADHD Prescription Medication Use; NCAA MANDATORY Requirements

• The NCAA has strict guidelines regarding medications for the treatment of ADD/ADHD; they are considered performance enhancers and BANNED unless proper exemption paperwork is maintained. If you currently take a prescribed medication, select the appropriate link below for further instruction.

    If you are brand new to Catawba College, please select this link for specific instructions as to what is required when you first come to campus; this is MANDATORY.

    If you have already submitted the information in the “New to Catawba” ADD/ADHD instructions, please select this link for specific instructions as to what is required for EVERY DOCTOR APPOINTMENT you obtain an ADD/ADHD medication prescription refill as long as you remain a student-athlete; this is MANDATORY.
